Liability Release/Confidentiality Agreement
By signing below, I agree not to disclose any client names, treatment information or identifying information pertaining to any client, past, present or future, of Alane Freund, Licensed Marriage & Family Therapist (LMFT36077), Sara Moss, Associate Marriage and Family Therapist (AMFT141374 Supervised by Yvonne Beyer 36544), Heart and Mind Equine (hereafter referred to Heart & Mind Equine) to anyone not affiliated with Heart & Mind Equine. This confidentiality agreement is effective on the date of the signing of this agreement and is forever binding after my association with Heart & Mind Equine ends.

Our cancellation policy is:
- Cancellations prior to 7 days before event will be refunded minus $25 administration fee
- Cancellations 1-7 days prior to the event will be refunded at 50%
- Cancellations in the last 24 hours prior to the event will not be refunded
